What to Bring

You can fill out and submit patient registration forms online before your appointment. If you do not submit forms in advance, we ask that you arrive at your appointment a few minutes early to complete the paperwork. Please bring the following with you to the consultation:

  • Referral slip, if applicable
  • Valid form of ID, such as a driver’s license
  • Form of payment
  • List of current medications
  • Dental/medical insurance information
  • Images or X-rays from your general dentist or doctor, if applicable

Parents of Minors

Patients under the age of 18 must be accompanied by a parent or legal guardian to the consultation appointment.

What to Expect

At the consultation, we will examine your teeth, explain your treatment options, and create a custom care plan for your dental needs. We will likely take 3D scans of your mouth, which are used to detect hidden oral health issues and create detailed surgical guides. It’s a good idea to write all of your questions about treatment beforehand so we can address them and make sure you feel informed about your options and upcoming procedure.

We use state-of-the-art CBCT scanning technology to view detailed images of the oral and maxillofacial region, resulting in better treatment planning and more predictable results.
